Immunization Needs for 2016-2017 School Year

To the parents of current 6^th grade and 11^th grade students:

The Ohio Department of Health has mandated that students entering the 7^th or 12^th grade this Fall (for the 2016- 2017 school year) must have the Tdap Booster and Meningococcal Vaccine (current 6^th graders) or Meningococcal Vaccine (current 11^th graders).

The forms for the vaccines were mailed to the homes of 6^th and 11^th graders. In case you did not receive a form, they are posted under the Forms and Links section near the top-right of the website. For your assistance, click below to download:
