Meal Order Form, October 12-16

Grab & go pick up will be available at each school daily from 10:30 - 12:30. 

Please complete this form FOR EACH CHILD that you plan to pick up meals for; for example, if you have 3 children in the district, you should fill out this form 3 times. You only need to complete this form ONCE PER WEEK PER CHILD. 

Upon pick up, you will be asked to provide the names of the students, their teacher & grade level. Their identity will be verified by a district employee. 

If you have students in different schools throughout the district, you will only have to go to ONE school to get all of your meals. If you have at least 1 high schooler, select the high school; if not, choose the school closest to you that your child attends.

Students will receive the main entree, fruit, vegetable, milk, and any other "extras" listed.

THIS FORM MUST BE COMPLETED TO GUARANTEE A MEAL FOR YOUR CHILD.
