The Educational Service Centers (ESC) of Northeast Ohio along with Giant Eagle are holding a Phizer Vaccine Clinic available to all students 16 years and older as well as to ALL family members 16 and older.
All vaccines will be given by Giant Eagle at the ESC of Northeast Ohio, located at 6393 Oak Tree Blvd., Independence, Ohio. Please be see other important information below.
- If you are under 18 years of age, a parent or guardian MUST be present for you to receive your vaccine.
- Please complete the School COVID Clinic Form attached.
- Print the form double-sided.
- Complete the entire front side, except the insurance section.
- Please bring your insurance card(s)
- If your medical and prescription insurance are separate cards, please bring both.
- Please bring proof of your date of birth
- Driver's license, state ID, birth certificate, etc.
- Please do not arrive more than 5 minutes early for your appointment.
- Second Dose:
- These appointments are for the FIRST dose only of the Pfizer vaccine. You will return at the same time on May 25th for your second dose.
- Please do not lose your vaccination card you receive at this appointment. You will need it for your second dose.
- You will also receive a new Clinic Form by email to complete prior to your second dose appointment.
